State clearly the information you are seeking eg place and date of marriage
Provide the information known and the supporting evidence eg birth certificates of both parties, place and date of children's birth and certificate(s), death certificate(s)
Copies of supporting evidence can be mailed, faxed or emailed as an attachment to the GSV.
